The simple, textbook solution puking underwater is to leave your regulator in your mouth. This reduces the chance of inspiring water while vomiting.
If chunks get caught in the exhaust ports, and a purge/shake doesn’t work, you may need to switch to an octopus if the regulator is breathing wet or flooding.
