As long as you have the hose orientation the way you need it, there’s technically not an “upside down” for a 1st stage dive regulator. Usually cave or penetration divers will prefer a more “tucked down” or streamlined position, but even then sometimes it makes little to no difference depending on the manufacturer.
The main concerns are hose routing, different valve configurations, comfort (not bonking your head haha), and keeping it streamlined.
